Patient must have been given oral sedatives at home that were picked up ahead of the appointment. This will assist in the comfort of the lion cut process to create the least amount of stress possible.

Please note: IM sedation will only be recommended if it is in the best interest of the pet as well as the safety of our staff. If selecting no for sedation, you understand that, should it be determined that the lion cut cannot safely proceed without sedation, the appointment will be ended and you will be asked to pick up your pet.
In signing this form, you affirm your understanding that payment is due at the time of service. Should any additional services be required aside from the above outlined items, payment shall also be due for these at the time of service. You certify that your pet is up to date on all required vaccines or, if not up to date, agree to update these vaccines while the pet is under the care of Orchard Road Animal Hospital.
